Project: Cutting Edge Interventional Radiooncology Optimization Software

The goal of the project is to develop and provide a software application for the pre-planning and dynamic execution of Ultrasound image-guided interventional brachytherapy procedure with a radiobiological optimizer along with organ library with radiobiology models and model parameters and radiobiology evaluation tools and inverse planning based on radiobiological optimization. _x000D__x000D_Currently, there is no other commercial or public software that integrates a radiobiological optimizer for brachytherapy planning._x000D__x000D_Brachytherapy is a method of cancer treatment, which has been used for over a century. Radioactive sources are placed manually or automatically, computer-driven (afterloading method), inside the cancerous tissue and positioned in a manner that will attack the cancer efficiently. Some of the diseases now treated with brachytherapy include prostate, cervical, endometrial and breast cancer as well as coronary artery disease. Brachytherapy has been proven to be very effective and safe, providing a good alternative to surgical removal of the prostate, breast and cervix, while reducing the risk of certain long-term side effects._x000D__x000D_The most important cancers treated by brachytherapy are:_x000D_(1) Prostate cancer: Represents by far the most frequent cancer for men in Europe, USA, Canada and developed countries. The SMEs of the consortium (Pi Medical & Medcom) delivered in 2002 a unique system for real-time, image guided intraoperative treatment planning and delivery, named Oncentra Prostate (OCP), CE marked and FDA approved. Until November 2008, more than 70 clinics in 20 countries worldwide are using this product and more than 2.300 patients will be treated with this prostate-specific system within 2008 _x000D_(2) Gynaecological tumours: Although in decreasing importance in developed countries, have enormous epidemiological importance for developing countries such as India, China & Brazil, as well as for third world countries. Consortium SMEs (Pi Medical & Medcom) delivered in May 2008 a unique system for dynamic, image guided intraoperative treatment planning and delivery, named Oncentra GYN (OCG), CE marked and FDA approved. About 10 such systems are sold, where the expected number of sold systems within the next 4 years is over 50._x000D__x000D_(3) Breast cancer: It is the most important cancer entity for females in Europe, USA and Canada, where 10% of all women will develop breast cancer. Due to difficulties in dynamic imaging of breast tissue and deformations occurring during intervention, there is no commercial technology available for that until now. However there is considerable clinical experience & expertise within the consortium on such type of treatments. _x000D__x000D_(4) Solid primary tumors and metastases in brain, lung and liver: Represent also a statistically significant application field for interventional radio-oncological procedures. For all these cancer localization, there is considerable clinical experience and expertise within the consortium._x000D_Currently, only site specific solutions (prostate & gynaecological cancers) offering pre- & dynamic planning capabilities are commercially available. According to our experience and market response, there is an increasing interest and need for a universal system for pre- and dynamic planning of interventional radio-oncological procedures. _x000D__x000D_The new product will integrate our existing organ-specific solutions (i.e. OCP & OCG) and furthermore will integrate:_x000D_- Advanced rabiobiological optimizer for dose calculation and needle placement_x000D_- Inverse planning based on radiobiological models_x000D_- Organ library with radiobiology models and model parameters_x000D_- Radiobiology based evaluation tools_x000D_- Handling of multiple imaging information resulting from CT, MRI and 2D & 3D U/S as well as any combination of those in a framework of multi-modal imaging_x000D_- Provide sophisticated tools for optimal needles path determination and 3D dose distribution calculations at any body site

he CO results of our CEIROS project was the development of a new, cutting edge software product with an advanced radiolobiological optimizer (gEUD based) for the pre-planning as well as integrated tools for the evaluation of radiation plan based on radiological model. In addition our software solution also offers a database application for storing and retrieving well known evaluation protocols._x000D_The final software product of CEIROS includes:_x000D_1) New optimization algorithm based on gEUD and advanced imaging tools (embeeded into Oncetra Prostate)_x000D_2) Evaluation tool (BIOSPOT)_x000D_3) Library of medical treatment protocols (BIOTOP)
