Project Topic The H@H project aims at solving societal problems related to healthcare services for elderly citizens affected by Chronic Hearth Failure (CHF), providing them with wearable sensor devices for monitoring of pathophysiological cardiovascular and respiratory parameters and, at the same time, enabling the medical staff to remotely supervise their situations and taking actions by involving public/private healthcare organizations. In Europe around 14 million citizens is affected by CHF with 3600000 new cases/year, representing the most common cause of hospitalisation in elderly persons. Previous experiences used to record weight, heart rate and blood pressure, using then telephone as data transmission support. Novel features of the H@H project, enabled by the developed ICT components, extend this approach by: (i) long-term monitoring of clinical parameters; (ii) new user-friendly systems to collect such data at home and (iii) new methods of data transmission and analysis. Prognostic prediction, patient quality of life, education and self-care, and cost-effectiveness in three European Countries (Italy, Spain and Slovenia) will be assessed, focusing on elderly people. As a final result, the project will develop the following prototypes: (a) a software platform for the recording and management of medical information for elderly affected by CHF taking into account emerging international standards (HL7-RIM and CEN 13606), (b) a monitor-based on ?Expert System? solutions that will serve to visualize the level of criticality of the patient, (c) a comfortable and easy to use integrated system (e.g. wrist watch) sensing main pathophysiological signals and able to provide data for tele-diagnosis to the centralised expert system. The proposed consortium has all the competences to face the ICT challenges. End-user will be involved both as elderly patients and as HealthCare Organization, both will be keys for user?s requirements definition and demonstration phases.
