Project Topic 1. Challenges: Main impairments of Quality of Life (QoL) for 65+ and families as well as major social costs increasingly result from Chronic Diseases (NCD). Ambient Home Health Care (HHC) Care is an effective way to increase Patient QoL. Informal HHC is already an important factor to close the gap between formal ambient care demand and supply and is expected to increase. ICT- enabled care management systems can effectively support Informal Caregivers. 2. Target groups: Informal Caregivers and NCD-Patients; public and healthcare stakeholders in general (dissemination) 3. Technology: Prototype of Self-Management System for NCD (web-based platform for PC and mobile devices (tablets, smartphones) plus wireless connected monitoring devices for main biomarkers (kg, mmHg, cal, mmol, %SpO2, FEV1) 4. Business model: a) commercial: System lease or Sale of hardware system plus quarterly service fee b) organisational: Revenue-share (license) 5. End user participation: Four national (CH, PL, ES, AT,) user groups (Informal Caregivers plus Patient) to be recruited by participant organizations ; one established national end-user organisation (Federation of Polish Patients) as participant.
