Project: The Real-time Drug Optimal Compliance Project

Patient non-compliance with prescribed therapies is a global issue of major public health concern. This is reflected in a recent report by the World Health Organisation (Adherence to long-term therapies: evidence for action. WHO 2003), which calls the non-compliance “a worldwide problem of striking magnitude”. In such circumstances, no further improvement in the effectiveness of therapeutic and prophylactic medication can be realised without addressing patient non-compliance._x000D_ The project will develop an intelligent communication system that is easy to use for patients and healthproviders. _x000D_ _x000D_ It will enable :_x000D_ _x000D_ - to collect centrally in real-time time drug intake data by patients, _x000D_ - to process and analyse automatically the collected information,_x000D_ - to report the results to patients and physicians, and when necessary under the form of a reminder or alert.with a message including recommendations or actions to be taken. _x000D_ _x000D_ Thus the DOC Project will directly enhance patient compliance and COtain long term persistence of treatment for chronic conditions._x000D_ _x000D_ The proposed system requires to develop:_x000D_ _x000D_ - an intelligent and autonomous decision-making analysis software (Medical Intelligence Suite) to process permanently the collected information_x000D_ -a bidirectional communication system (Collaborative Platform) to take action according to pre-defined scenarios_x000D_ _x000D_ In the original proposal, the primary goal of ABR Pharma was to develop a communicative smart sticker to automatically compile drug dosing history data with blister technology. Unfortunately, ABR Pharma encountered financial difficulties that forced him to leave the consortium. Therefore the two reCOing Ps proposes to replace the technology that was supposed to be developed by ABR Pharma with a technology available from the AARDEX Group. It is a communicative device that collects and communicate immediately the moment (date and hour) when the patient takes each individual pill out of a medication container. This technology is very similar to what was planned to be developed in the original proposal but does not apply to a blister package. _x000D_ _x000D_ _x000D_ The Medical Intelligence Suite (MIS) is a software platform including : _x000D_ -a web service, an exchange protocol allowing :_x000D_ ° to transfer data from compatible compliance devices to a centralised repository ;_x000D_ ° to acces different views on patient compliance._x000D_ -a standardized database to store dosing history data and related clinical data._x000D_ -a set of algorithms supporting the interpretation of drug dosing history data in different clinical settings._x000D_ _x000D_ The Collaborative platform is a communication system that will manage compliance program participants (patients, health care providers) and will communicate with them on data, information, decision, advice, and alert messages based on pre-defined scenarios and according to the input provided by the MIS._x000D_ _x000D_ A real case scenario: (the type and management of the data will depend on the medicine to be taken, the pathology and the healthcare network involved). The patient’s doctor identifies itself on the collaborative platform, gives the patient a preset kit including electronic smart package and a GSM modem. The Real-time DOC solution is then ready to operate. The Smart package starts sending intake data to the Medical Intelligence Suite that will process the data. When necessary, according to results provided by the MIS, the Collaborative Platform communicates via automatic text messages or phone calls , for example to raise its awareness on possible forgetting, on case of overdose or detected emergency situation. This communication will follow a pre-defined scenarios and can be done towards patients or even patient’s doctor should the situation require its intervention._x000D_ _x000D_ To achieve such ambitious objectives, the consortium gathered three complementary Ps with international recognition in their respective fields of knowledge: _x000D_ -Citobi, a Belgian firm specialised in patient and healthcare providers relationship management solutions._x000D_ -AARDEX Group, a Swiss firm that is the world leader in interpreting drug dosing histories in various clinical settings._x000D_ -The University-hospital of Lausanne, that has developed a worldwide recognized expertise in conducting research in patient compliance._x000D_ _x000D_ The final Real-time DOC Project achievements will significantly impact our public Health system thanks to an enhanced safe-effective and cost-effective use of medicines and a general improvement of patient health and quality of life.

At the end of the project, the participating companies have built a fully working prototype of a complete suite of tools enabling to set up monitoring programs for patients under treatment for chronic diseases._x000D__x000D_The suite is composed of_x000D__x000D_- A wireless intelligent medication container that can communicate with a central server to transmit drug dosis events._x000D_- A drug history analysis software that is able to record and display data collected by so-called "smart devices" and to detect specific non complance patterns._x000D_- An interactive communication platform allowing to manage communication different channels between doctors, patients and pharmacians.
